Mobile Medical Update: Effective July 1, 2024

Mobile Medical Update: Effective July 1, 2024, the we will be switching to electronic Annual and Random notifications. Please contact Mobile Medical Corporation (440) 895-8358, by June 28, 2024, to ensure MMC has your correct cell phone number and email for all future annual and random testing notifications. HRA Card Changes

Pro-Flex, the Fund’s third-party administrator that handles the HRA account, has been acquired by Benefit Resource (BRI). Participants will receive new cards in late May/early June and a blackout period for cards and claims will occur from June 1st-June 10th. Watch for new cards and further information to be mailed directly from BRI.

Dental Plan Changes

Please Note: Dental claims are processed through the fund office. DO NOT PAY OUT OF POCKET OR
GIVE THEM YOUR MMO/ HUMANA CARD. Claims must be mailed or faxed (216-525-2002) directly
to the Fund Office, under the policy holders name & Social Security number not to the medical insurance
